In Customize, on the Lead Generation tab, set Contact method to Contact form. A Template setting appears.

Request a quote for businesses that price per job.
Book a consultation for appointment based services.
Custom to build your own.
The first three are fixed. They are sensible defaults and you cannot change their fields, which keeps them predictable.
Build a custom form
Choose Custom to unlock two extra settings: Form title and Fields.
Form title
Add a title of up to 80 characters. This is the headline visitors will see, so make it about them: “Tell us about your project”, not “Lead capture form”.
Fields
Every form includes Name and Email by default. These cannot be removed.
You can add up to 10 additional fields. Each field can be one of these types:

Each field can also be marked Required or Optional.
Keep it short
Shorter forms get more submissions. Extra fields add friction, so only ask for what you need to make the first follow up useful.
For most businesses, that usually means:
- Name
- Phone
- A short description of what they want
Where submissions go
Submissions go straight into Leads, along with the form name, so you can see where each lead came from.
They are also emailed to the people you specify. See Manage leads.
Spam protection
Forms are protected automatically. You do not need to configure anything.
